The danger is not simply that a client forgets to mention a medication. The deeper problem is that without a structured record, you cannot show what was asked, what was found, why treatment proceeded or was postponed, what the client was told, which settings and products were used, or what happened afterwards.

That is why microneedling intake forms matter. They are not admin for its own sake, and they are not a legal shield. They are one part of a professional decision-making system that connects medical and skin history to contraindication screening, informed consent, the treatment record, written aftercare and follow-up. A form records the process; it does not replace competence or judgement.

The documentation, layer by layer

The intake, consent and disclaimer pages are three understandable layers — but a complete record needs more than three forms. It should also capture the treatment record, the aftercare given and the follow-up.

Intake & screening	Relevant medical, medication, allergy, skin and procedure history; current products; pregnancy/feeding where relevant; the treatment goal. It collects information — it cannot diagnose outside your scope or decide suitability by itself.
Assessment & decision	Observed skin, pigment/scar history, and the decision to decline, defer, refer or proceed, with the reason. A ticked box cannot replace judgement or device guidance.
Informed consent	The procedure, material risks, benefits, alternatives, likely recovery, questions, a voluntary decision and the right to withdraw. A signature cannot make an unsuitable treatment suitable or remove your duty of care.
Treatment record	Date, device, cartridge, lot where available, area, actual parameters, endpoint, products and anything unexpected.
Aftercare & follow-up	Written instructions, warning signs, contact route, follow-up and any adverse event. Link clients to the aftercare guide rather than a fixed calendar.
Privacy & records	Purpose, minimum-necessary collection, access control, security, correction, and lawful retention and disposal under POPIA.

What written records can — and cannot — do

Records matter because memory is unreliable and disputes are decided on evidence. But a form cannot excuse poor screening, treatment outside your scope, inadequate hygiene, unsuitable parameters or a failure to respond to a complication. It evidences the standard of care only when the underlying process was sound. A disclaimer is not a waiver of client rights, and a signed form is not automatic insurance cover — check your own policy wording, exclusions and scope with your insurer or broker.

South African privacy (POPIA)

Medical history is special personal information. POPIA needs more than a consent sentence on a form: identify why you collect the information, collect only what is necessary, control access, keep it secure and accurate, explain the client’s rights, and follow a lawful retention and disposal process. Consent in South Africa is centred on information and participation — explained in language the client understands — not merely a signature.

Why I built this system

My background is investigative journalism, where records, timelines and exact wording matter. Moving into skincare education and formulation, I saw the same gap repeatedly: practitioners performing a procedure with no consistent record connecting the client’s history to the decision, the consent discussion, the treatment and the follow-up.
