Microneedling Consent Form

A microneedling consent form documents informed consent — a real discussion between practitioner and client, not just a signature. It records that the client understood what was proposed and chose to go ahead voluntarily. It does not, on its own, make an unsuitable treatment acceptable or remove your duty of care.

What a microneedling consent form should contain

• The procedure explained in plain language.
• The material risks — infection, post-inflammatory hyperpigmentation, bruising or pinpoint bleeding, scarring or track marks, and cold-sore reactivation.
• The expected benefits, stated realistically, and that results vary and often need a course of treatments.
• The reasonable alternatives, including the option not to proceed.
• The likely recovery and aftercare responsibilities.
• An opportunity to ask questions, a voluntary decision, and the right to withdraw consent at any time.

Consent is a process, not a signature

In South Africa, informed consent is centred on information and participation — explained in language the client understands — not merely a signed page (National Health Act). A signature cannot make an unsuitable treatment suitable, remove your duty of care, or guarantee that an insurer will pay. Whether to treat at all is a separate decision: see decline, defer, refer or proceed.
