Microneedling Disclaimer Form

A microneedling disclaimer (or acknowledgement) form records that the client understands the realistic limits of treatment and agrees to their part of the aftercare. It is a useful record — but it is not a liability waiver. It does not remove your duty of care, excuse negligence, or protect you from a claim arising from treatment that should not have gone ahead.

What a microneedling disclaimer form can record

• Realistic expectations: results vary, improvement is gradual, several sessions may be needed, and no procedure can promise complete scar removal or permanent pigment clearance.
• Client-supplied information: that the history and medications they disclosed are accurate and complete to their knowledge.
• Agreed responsibilities: the aftercare the client agrees to follow — sun protection, avoiding heat and picking, and contacting you if warning signs appear.

What it cannot do

A disclaimer does not waive the client’s rights and does not transfer responsibility for an unsuitable treatment, poor screening, inadequate hygiene or a failure to respond to a complication. The effect of any clause depends on the facts and the law, so have your documents reviewed for the services you actually provide and your jurisdiction.
